Cedars-Sinai C.A.R.E.S. Program

The C.A.R.E.S. Program, at Cedars-Sinai's Jona Goldrich Center for Alzheimer’s & Memory Disorders, provides people living with Alzheimer's or other forms of dementia with evidence-based care designed to help them and their families navigate medical, behavioral and social challenges.

A diagnosis of dementia can be challenging for the patient and the entire family. Our multidisciplinary team understands this and draws from expertise in neurology, geriatrics, nursing, social work, neuropsychology, and palliative care to provide comprehensive care, resources, and support for patients and caregivers.

Our nurse practitioners, who are experts in dementia care, identify the needs of each patient and their caregivers and guide them on symptom management, caregiving challenges, advanced care planning, and accessing vital resources. In collaboration with the patient's primary care physician, the nurse practitioners develop a personalized care plan to meet both the present and future needs of the patient and their family.
