Zaldy S. Tan, MD, MPH

Zaldy is an aging and memory disorders specialist in Los Angeles, CA and affiliated with Cedars-Sinai Medical Center. He is the Director of the Cedars-Sinai Health System / Memory & Aging Program and the Medical Director of the Jona Goldrich Center for Alzheimer’s & Memory Disorders at the Cedars-Sinai Medical Center. Dr. Tan’s research in memory and aging has been featured in the New York Times, Wall Street Journal, CNN and other national publications. He trained at Brown University and Harvard Medical School. He is experienced in Alzheimer’s disease, preventive medicine, medical education, memory disorders, and aging.

Deana Rhinehart, DNP, AGNP-BC

Deana has been practicing as an adult gerontology primary care nurse practitioner for over 8 years. She earned her Doctorate in Nursing Practice degree from East Tennessee State University. Deana has dedicated the last 20 years of her nursing career to service the older adult population in various clinical settings. She understands the challenges faced by people living with dementia and their caregivers and is committed to providing quality care and support. She is passionate about her work and strives to improve the lives of her patients and their families.

Dyane Gatmaitan, MSN, AGPCNP-BC

Dyane has been an adult geriatric nurse practitioner for over 10 years. She earned her advanced practice nursing degree at UCLA and has dedicated her nursing career to serving the adult and older adult population in various settings, from home health to clinic-based care. She strives to support her patients and their families with empathy and patience as they navigate the challenges associated with dementia.

Gabriela Torres, CCMA

Gabriela has been a dedicated certified medical assistant for the past 20 years. She received her certification from Northwest College and began her career working in family practice. She feels a deep desire to help people facing the challenges of dementia and is committed to providing care with understanding and kindness. Building meaningful relationships with patients and their families motivates her to continue working in dementia care. She is passionate about directly improving patients’ quality of life by providing personalized care and creating a safe, supportive environment for both patients and their families.
